All rebate offers are only for annual and semi-annual orders that are shipped directly from the manufacturer to the end
user. Orders must meet the new eye exam requirement along with the quantity of boxes the manufacturer has listed on
the rebate pages in order to qualify for the rebate. All qualifying orders for rebates must be on the same invoice. Orders
cannot be combined to meet the quantity required for a rebate. The manufacturer ships the rebate coupon direct from the
factory with the qualifying orders.
In order to qualify for most manufacturer rebates a new eye exam is usually required within
